Smolder Uploads Not Generating Proper Response to Submitter

I have been getting similar results on both Ubunti 9.10 amd64 and Ubuntu
9.10 i386.

I have had to go to the Website to check if the report has been submitted,
http://smolder.plusthree.com/app/public_projects/smoke_reports/8

I have had to resend it only once.
