More IO Changes (need feedback)

This is work that I had wanted to do ever since I started io_cleanup1.
I hadn't gotten around to it in part because I was taking a bit of a
break and in part because I didn't think it was urgently needed. brrt
pointed out to me last week that io_cleanup1 had indeed broken
mod_parrot, and these branches should fix his issues.

These branches are all still very very early in development and I do
not plan to merge them any time soon. Threads should happen first and
I will work around that. I also need some serious feedback from Rakudo
about the FileHandle/Pipe thing before I do anything.

Threads is first. If there's anything I can do to help that at this
point, let me know.
